I just beat Playful Darkness on Unfair and didn't get the achievement. The title pretty much says it all. I dropped the difficulty down to core and I still didn't get the achievement. I just got another achievement on the same save file a couple days ago. Save files created before the patch may have achievements disabled or bugged.

On my Core playthrough swarms did good damage. In fact, they killed Darkness by themselves, with the rest of the party just missing every attack.
